Outline of Research at the Start

嚥下運動は中枢性にも反射性にも誘発可能な半自働運動として知られている.随意性嚥下に関わる上位脳の局在や時間的・時系列的な活性化については,ヒトにおいてのみ研究されてきており,外側中心前回や中心後回,島皮質に加えて,前帯状皮質が嚥下前から活性化することが報告されているが,中枢性嚥下に対して,前帯状皮質が時間的・空間的にどのような関わりをもっているかについて追及した研究は全くない.そこで本研究では,ラットを用いて,神経生理,薬理,解剖学的アプローチにより,前帯状皮質の嚥下誘発効果検証,嚥下運動時前帯状皮質の活性化記録を行うことで,嚥下運動誘発に関わる前帯状皮質の働きを明らかにする.

Outline of Final Research Achievements

The effects of stimulation applied to the anterior cingulate cortex (ACC), which are reported to be involved in inducing swallowing, were investigated in anesthetized rats. When the number of swallows in 10 seconds was compared between rats with and without the ACC using low threshold electrical stimulation of the superior laryngeal nerve to induce swallowing, the effects of ACC stimulation varied between animals. Histological findings showed no significant differences between the effective and ineffective groups for ACC, and there was no after effect of neural activity due to repeated stimulation. Although several human studies have reported the involvement of the ACC in voluntary swallowing, the ACC, which is involved in the reward system, is thought to be active as a reward system obtained by occurrence of swallowing reflex, rather than being related to inducing swallowing.

Academic Significance and Societal Importance of the Research Achievements

現在,摂食嚥下障害の臨床における興味の焦点は運動障害に対する理学療法的なアプローチや対症療法,さらに障害に対応する食品開発などに置かれているが,摂食嚥下障害の主たる問題のひとつである「嚥下運動誘発障害」にも着目しなければいけない.本研究では,ヒト研究での報告をもとに基礎的研究を行った結果,島皮質とは異なり前帯状皮質の嚥下誘発能に関する明らかな効果は得られなかったものの, 今後は慢性実験にてマルチ電極を用いた網羅的な記録により,嚥下誘発または嚥下運動に伴う前帯状皮質の活性化と臨床技術への応用に向けた研究アイディア構築に努めたい.
