Outline of Research at the Start

腹膜播種を伴う進行卵巣癌に対する有効な治療法は未だ確立されておらず、腫瘍の病勢を制御することで難治性を打破する新たな治療戦略が期待される。進行卵巣癌腹膜播種において、卵巣癌細胞と腹膜中皮細胞との異種細胞間クロストークの重要性が明らかとなってきた。本研究課題では、脂肪組織近傍に存在する腹膜中皮細胞の特性と卵巣癌細胞の接着・親和性との分子生物学的関連を探索するため、ヒト大網由来の腹膜中皮細胞および脂肪細胞を使用した共培養モデルや、腹腔内における腹膜中皮細胞特異的に標識した遺伝子改変マウスを用いることで、卵巣癌-腹膜中皮細胞間の接着分子と細胞外基質との相互作用に着目した分子機構を詳細に解明する。

Outline of Final Research Achievements

Effective therapies for advanced ovarian cancer with peritoneal dissemination remain limited, and novel strategies are urgently needed. We previously reported that normal peritoneal mesothelial cells, which usually play a protective role, can be transformed into tumor-promoting ovarian cancer-associated mesothelial cells (OCAMs) in the tumor microenvironment. We also demonstrated that vitamin D suppresses this transformation and restores normal cell function. In this study, we aimed to identify vitamin related compounds with stronger efficacy and fewer side effects for potential clinical application. Furthermore, to discover new therapeutic targets, we conducted marker screening to distinguish normal mesothelial cells from OCAMs. Through this process, we identified a novel molecule, termed Target Molecule X, and investigated its role in OCAM function.

Academic Significance and Societal Importance of the Research Achievements

卵巣癌は他の癌腫と比較して免疫チェックポイント阻害薬の有効性が乏しく、新規治療戦略の開発が期待されている。癌そのものを標的とした治療ではなく、癌微小環境を標的とした腹膜環境の正常化は卵巣癌腹膜播種の新規治療戦略となる可能性がある。その候補の一つにビタミンDを報告しているが、本研究では臨床応用に向けて、効果が高く副作用の少ない治療薬候補としてビタミンD関連化合物の有効性を検証した。今後、副作用の発現の検証と臨床的な効果の検証が期待される。また正常中皮細胞とOCAMを識別するためのマーカーを同定した。本研究成果を基盤として、腹膜中皮細胞を標的とした卵巣癌治療の確立を目指す。
