| Outline of Research at the Start |
昨今、リアルワールドデータ（RWD）を用いた研究が注目されている。本研究では、「生活習慣病に対する治療が、実際の住民集団において有効であるか」を、前向きコホート研究のデータと、医療・介護レセプトデータ等のRWDとを突合し縦断的に検証する。コホート研究の参加者を、RWDで定義された治療内容に基づき複数の群に分け、心血管疾患や骨折、要介護状態等のアウトカムのリスクを比較する。コホート研究のデータとRWDの両方を活用することで、医療の質向上や患者アウトカムの改善に寄与し得る、これまでは検証し得なかった様々な知見を得ることが可能である。

| Outline of Final Research Achievements |
We merged the data from the Nagasaki Islands Study (NaIS), a population-based prospective cohort study in Goto City (Nagasaki Prefecture, Japan) with drug dispensing data collected from pharmacies in Goto City. The dataset includes baseline data (lifestyle, variables on atherosclerosis, and osteoporosis screening data), disease registry data on myocardial infarction, stroke, and fractures, and drug dispensing data during April 2014 and June 2023, making it possible to examine the efficacy and side effects of many drugs, especially these for prevention from cardiovascular disease and bone fracture.

| Academic Significance and Societal Importance of the Research Achievements |
地域住民コホート研究で、詳細な処方データや治療内容等の情報の利用が可能な研究は希少である。多くのコホート研究では、対象者の薬剤歴、治療歴などの情報については、本人の自己申告に基づくため正確でないこともあり、活用に限界があった。逆にリアルワールドデータだけであっても、研究目的で収集されたデータでないため、身体所見や検査データについての情報はほとんどの場合、含まれていない。今回得られた研究基盤の活用により、これまでの研究では十分に検証し得なかった課題の解決が期待される。
