| Outline of Research at the Start |
超高齢社会において日常生活機能低下の予防策の確立のためには、低下のタイミングやプロセスを明らかにすることが、高齢者個人へのケアと政策の両レベルで重要である。本研究では、約10万人の高齢者の最大12年間の追跡調査により、1)高齢者の日常生活機能低下にはどのようなパターンがあるのか、2)高齢者の日常生活機能低下の各パターンと関連する個人要因は何か、3)高齢者個人の日常生活機能に影響を与える社会環境要因は何か、の3つを解明する。本研究により、終末期の日常生活機能低下の予防策を個人レベル及び社会環境レベルの両面から戦略的に進めるための、根拠資料を得ることを目指す。

| Outline of Final Research Achievements |
The purpose of this study is to identify patterns of functional disability among older adults and to elucidate the factors associated with these patterns. By analyzing longitudinal data from the JAGES project, we identified five distinct patterns of functional disability preceding death: the minimum disability pattern (46.3%), the catastrophic disability pattern (19.2%), the accelerated disability pattern (12.4%), the persistently mild disability pattern (12.6%), and the persistently severe disability pattern (9.4%). Additionally, we found that older adults who participate in social activities are more likely to be categorized under the 'Minimum disability pattern' after adjusting for potential confounders. Our findings suggest that policies promoting social participation could play a crucial role in supporting healthy aging.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、高齢者の社会参加が死亡前3年間に機能障害が少ないことと関連することを明らかとした。機能障害の発生だけではなく、その後の変化パターンに着目した研究は限られている。女性ではグループの種類に関わらず期待される関連が見られたが、男性では上下関係の発生しやすいグループ活動への参加で期待される結果が見られなかった。また、この関連は比較的若い高齢者において強く見られた。本研究により、社会参加は介護予防のみならず生活機能の維持とも関連を持つことが示唆されたが、性別や年齢によりその関連は異なることを示すことができた。高齢者を対象とした社会参加を促進する施策や事業などの根拠資料を示すことができた。
