Outline of Research at the Start

病院の部署の組織学習活動(EBP継続につながる集団の学習活動)により部署全体でEBPの意義や価値の理解が高まり、看護職はEBPを継続する可能性がある。また集団の組織学習活動は集団特性や文脈の影響を受ける。各部署が部署の組織学習活動を高める/低下させる要因を管理し、自発的に組織学習活動を行うことが望ましいが、EBP継続に向けた集団の組織学習活動の要因は明らかになっていない。
本研究では、集団の組織学習活動を高める/低下させる要因と条件を明らかにし、組織学習活動への影響を検証する。それにより、EBPを行う集団が組織学習活動を行うための条件が明らかとなり、組織学習活動促進に向けた実装戦略の構築につながる。

Outline of Final Research Achievements

This study aimed to identify the factors that promote or inhibit learning activities in hospital wards, which could be effective for nurses' sustainable implementation of evidence-based practices (EBPs).
We conducted focus-group interviews with healthcare professionals involved in implementing EBPs in the wards. By conducting a content analysis of the axis of the organizational change framework, 45 items were identified across eight categories: sociopolitical context and healthcare policy (e.g., demands from society), style (e.g., a psychologically safe climate), superordinate goals (e.g., nurse managers' vision), strategy (e.g., nurse managers' strategic thinking), skills (e.g., doctors' soft skills-communication skills, creativity, open-mindedness), staff (e.g., professionalism), systems (e.g., a system for the nursing department to support and instruct the EBP-implementing departments/wards), and structure (e.g., a system for flexibly reviewing the allocation of duties).

Academic Significance and Societal Importance of the Research Achievements

本研究は看護職向けの実装戦略の開発に向けた基礎研究として、組織変革の枠組みを基に、部署の学習活動の規定要因を組織内外の階層と多様なステークホルダーを含めて明らかにした。既存の実装戦略では、看護職が組織の状態を把握したうえで実践の場に適した戦略を活用することは難しかった。本研究は、組織の状態を体系的且つ包括的に提示しており、組織の管理者や看護職が組織の状態を把握する枠組みとして本成果を活用することで、部署の学習活動が活発になり、新しい実践の継続と組織への定着が促進できる可能性がある。また本研究は、組織論を基に実装戦略の開発を目指す点が新しく、理論への貢献も期待できる学術的意義の高いものである。
