| Outline of Research at the Start |
本研究の目的は、研究1として、DELTAプログラム導入下の入院患者のベッド位置や移
動、術後の部屋移動、日中の自室外への外出などの環境要因とせん妄発症との関連を後方視 的な方法で明らかにする。研究2では、病棟看護師の実践において、明確な根拠はないもの の、せん妄のきっかけとなる状況や要素に関する臨床の気づきのなかから、せん妄を誘発す る可能性のある病棟環境の要因をグループインタビューにてリスト化することである。リスト化後はせん妄発症と の検証を経て「せん妄予防のための病棟環境整備ガイドライン」への発展を目指す。

2022年度に金沢医科大学病院と連携し、整形外科病棟に入院し、全身麻酔による手術を受けた患者のうち、DELTAプログラムのSTEP2でせん妄症状ありにチェックされた患者を対象とした。せん妄リスクと療養環境の関連を明らかにするために、レトロスペクティブに調査を行った。結果は、対象者が43人（うち女が24人）、年齢の中央値が78歳（44-92歳）だった。年齢と性、入院からの手術までの日数を考慮しても、術前部屋移動はSTEP２せん妄症状に関連する可能性が示唆された。2023年度、分析結果をmedicineに投稿中。
