Outline of Research at the Start

本研究は、薬剤のみで発作コントロールが困難なてんかんである、薬剤抵抗性てんかんの外科治療の治療成績を向上させることを目的とした研究である。治療成績の向上のためには、正常機能とてんかん原性領域を正確に同定し、正常機能を担う脳部位を極力温存し、てんかん原性領域を極力切除することが重要である。本研究は、術後発作完全消失のために、てんかん原性領域をどのように正確に同定するかに焦点を当てた研究である。具体的には、発作間欠期の頭蓋内電極脳波データを新手法で定量化し、てんかん原性領域の正確な同定の指標として有用であるかどうかを調査する予定である。

Outline of Annual Research Achievements

本年度においては、昨年に引き続き、3施設すべてにおける脳波データ、画像データの取得と、脳波データのプレプロセシング、画像データから各患者における頭蓋内脳波電極位置の同定、それらを標準脳に変換するという作業を行い、データを解析可能な状態に加工した。また、切除領域内かその外かを同定するために、術後MRIも取得しそれらの同定も行った。さらに、臨床情報の取得も行うことで、多変量解析などに必要な情報を整理した。加えて、昨年国際学会で発表したテーマについての追加解析と論文執筆へ向けた準備を行った。また、複数のバイオマーカーを組み合わせて機械学習を用いることで、より精度の高い、術後予後予測モデルの作成に励み、これらの成果は2023年12月にOrlandで行われたAmerican Epilepsy Society Annual Meetingでポスター発表した。また、本演題は受賞するとともに、口頭発表演題にも選ばれた。

また、術中の頭蓋内電極データによる予後予測を行うために、術中脳波に関しても上記と同様の過程を行い、データの解析を行い、麻酔条件によって、バイオマーカーの変化がみられることを確認し、統計学的な解析もそれを支持するものであった。こちらも2023年12月にOrlandで行われたAmerican Epilepsy Society Annual Meetingでポスター発表した。

両テーマとも、学会発表にてたくさんの世界各国の研究者や臨床家から質問を受け、有意義なfeedbackを受けた

Reason

上記研究実績の概要に記した通り、解析を行い、その結果を学会で成果報告しており、おおむね順調に進展していると考えられる。予期していなかったこととしては、予測モデルの作成には、少し改善の余地があることがわかり、その追加解析のためのデータ見直しを必要としたため。
