Outline of Research at the Start

インスリンを分泌する唯一の細胞である膵β細胞の減少は, 糖尿病の病態の中核をなす. 本研究では, 非侵襲的膵β細胞イメージングを使用し, 緩やかに耐糖能が悪化する2型糖尿病モデルマウスにおいて, 糖尿病発症・進行過程の膵β細胞量の経時的変化を生体において明らかにし, その際の膵β細胞量と耐糖能の関係を詳細に検討する. 更に, 膵β細胞の病理学的検討, 遺伝子発現解析を組み合わせることで, 膵β細胞量変化を軸とした糖尿病の病態解明を目指す. また, 2型糖尿病の発症・進展に重要な役割を果たす肥満についても, オミックスデータを用いた解析行い, 2型糖尿病・肥満の代謝異常を統合的に検討していく.

Outline of Annual Research Achievements

2型糖尿病モデルマウスRCS10の糖尿病発症・進行過程における病態の変化を詳細に検討した。 具体的には、2型糖尿病モデルマウスRCS10において、糖尿病の発症および進行過程での血糖値およびインスリン分泌プロファイルの変化を解析した。結果、脂肪負荷食群では、肥満によるインスリン抵抗性の増加を反映する形で、血糖値が上昇する前にインスリン値が上昇し、その後、ある週齢を境に減少に転じ、以後経時的に減少していくことを確認した。また、インスリンが減少に転じる少し前から、血糖値が上昇し始め、その後経時的に上昇することも確認できた。 これは、肥満の悪化によるインスリン抵抗性の増大を、初期にはインスリンの過大分泌により代償することで血糖値を抑えるが、やがて代償機構が破綻し、経時的に耐糖能が悪化していくとされる, 2型糖尿病の自然史に合致した所見である。病理学的検討も進め、これらの血糖値およびインスリン値の変化に、病理所見上のインスリン分泌膵β細胞量(BCM)との関係を評価した。イメージングに関しては長期間装置の修理が必要となっていたが、完了後にβ細胞イメージングの撮影も終え、上記所見との関連も評価を進めている。更に、本研究の重要な因子である肥満に関連して、ヒトデータを用いて研究を進めた。具体的には、肥満が2型糖尿病、心血管代謝疾患、COVID-19重症化を引き起こす機序について、血中の4907のタンパクの中から、これらの関係を媒介する因子を、200万人以上のヒトゲノム、オミックスデータならびに遺伝統計学的手法を組み合わせることで解析した。これにより、肥満による2型糖尿病、心血管代謝疾患、COVID-19の発症リスク上昇を媒介するタンパクを同定し報告した(Yoshiji et al., Nat. Metab. 2023; Yoshiji et al., medRxiv 2023).
