Outline of Research at the Start

本研究では、身体活動経年変化による健康影響とその作用プロセスを解明することを目的とする。本研究では、1）中高年期の23年間における変化を類型化し、各パターンの認知症・要介護リスクを解明する。2）前期高齢期の身体活動変化と後期高齢期の身体・認知機能低下を媒介する要因を主観・客観的データを用いて解明する。本研究により得られる知見から、高年期の疾患発症リスクが高い、重点介入すべき集団を早期に特定することができるようになる。

Outline of Annual Research Achievements

当該年度は、オーストラリアのクイーンズランド大学が実施しているコホート研究（The Australian Longitudinal Study on Women's Health: ALSWH）のデータを用いて、高齢期女性を対象とした分析を実施した。本研究では、ALSWHのデータを用いて、高齢期女性の身体活動の経年変化パターンと身体機能との双方向の関連を検討した。解析対象者はオーストラリアの国民健康データベースから無作為に抽出された1921-26年に生まれたオーストラリア人女性8,238名とした。1999年（73～78歳）から2011年（85～85歳）まで3年間隔で郵送調査を行い、身体活動と身体機能に関するデータを収集した。73～90歳までの身体活動・身体機能の変化パターンを混合軌跡モデルを用いて検討し、両者の関連を制限付き3次スプラインモデルを用いて検討した。解析の結果、身体活動および身体機能それぞれに、Low、Moderate、Highに分類された。高身体活動群の女性は高い身体機能を維持し、低身体活動群の73歳時点の身体機能レベルに到達するのは87歳であった。同様に、高身体機能群の女性は他群よりも高い身体活動量を維持していた。低身体機能群の女性は研究期間を通じて身体活動量が極めて低く、身体機能は身体機能障害閾値を下回ったままであった。制限付き3次スプラインモデルでは、PAとPFが互いに影響を及ぼし合うことが示唆された。これらのことから、より早期から身体活動および身体機能を高める介入を行うことが、高齢女性の介護予防に有効である可能性が示唆された。
研究期間全体では中年期および高齢期における長期間の身体活動レベルの経年変化パターンを明らかにし、その規定要因および健康影響を検討した。これらは早期からの個別予防戦略に資する知見として活用されることが期待される。
