Outline of Annual Research Achievements

本研究では、臓器血栓塞栓症の予防のためにワルファリン導入が必要な者を対象に、「遺伝子情報によるワルファリン維持投与量の適正化が治療の効率化に繋がるか」を前向き臨床試験により検証することが目的であった。本年度は、昨年度に引き続き、被験者の登録を行い、遺伝子情報をもとにしてワルファリン初期投与量を設定して投与を開始し、PT-INR(プロトロンビン時間国際標準比)の推移をもとにして維持投与量に達するまでの期間、来院回数等を評価した。
また、新たなワルファリン維持投与量予測因子の探索として、以下2点を検討した。
①アフリカ系アメリカ人で有力な予測因子であると報告された(Perera MA et al. Lancet 2013)
rs12777823遺伝子多型を日本人で検討した。この遺伝子多型は、日本人ではワルファリン維持投与量の予測因子にはならないことを明らかにした。
②ワルファリン代謝酵素であるCYP2C9のプロモーター領域の3箇所の遺伝子多型(-2664-2665delTG、-1565T>C、-1188T>C)とワルファリン維持投与量との関連を検討した。一つずつの遺伝子多型の検討では、有意な関連は認められなかった。3箇所の遺伝子多型のハプロタイプとの関連を検討したところ、ワルファリン維持投与量が低いハプロタイプを特定することができたが、そのハプロタイプはCYP2C9*3型と一致していることが明らかとなった。
