骨折変形治癒に対する3次元評価システムの確立

Research Abstract

最終年度は、前年度に完成させた骨折変形治癒に対する3次元的定量解析システムを用いて、骨折変形治癒に対する3次元的定量解析を行った。撓骨遠位端および前腕骨折変型治癒、内反肘、脛骨近位および遠位部骨折変型治癒をそれぞれ40例程度を対象として解析検討した。予定より早期に解析が終了し、予想を上回る成果が達成できた。成果は、中部日本整形外科災害外科学会、日本手外科学会学術集会、67th Annual Meeting of the American Society for Surgery of the Hand、24th Congress of the European Society for Surgery of the Shoulder and Elbowで発表した。また本年度は英語論文7編が受理された。
これまで骨折後変形治癒において、その変形を正確に評価するシステムが未だに存在せず、3次元的な変形様式が明らかにされていなかったが、本研究の成果は、大きく以下の3点に集約される。(1)最新の医療画像及びソフトウェア技術を用いて骨折後の骨形状変化を3次元的高精度に定量解析するシステムを構築できた、(2)実際の臨床症例を対象に本システムを用いた形態解析を行い、様々な身体部位における骨折変形治癒の特徴的変形パターンが明らかになった、(3)変形パターンから想定される部位特異的な骨折後の変形メカニズムや変形治癒が隣接関節機能・荷重機能の障害を引き起こす病態が解明され、骨折後遺症としての骨折変形治癒に対する治療指針を確立につながったことである。また予想を上回る速度で研究が遂行でき、平成24年12月に研究を終了した。研究の成果は、整形外科領域で最も権威のある雑誌であるJ Bone Joint Surg Amに3編論文が掲載されたのをはじめ、いずれも権威のある雑誌に計15編の論文が掲載された。
