Research Abstract

特発性肺線維症などの進行性肺線維症や難治性喘息における気道周囲の線維化は、グルココルチコイドに対する反応が不良であり、新たな治療戦略が望まれている。代表者は、組織の発現レベルや生理的作用から、グルココルチコイド受容体、Proliferator-Activated Receptor (PPAR) –γとクラスターを形成する核内受容体Liver X Receptor (LXR)-αに注目をし、検討を進めた。その結果、合成・生理的LXRアゴニストは、in vitroで抗線維化作用を示す事を新たに見いだした。また、抗線維化作用は、PPARγ非依存的、SMAD非依存的な経路を介する事を示した。次に、マウスブレオマイシンモデルを用いて、線維化病態における役割を検討した。ブレオマイシン投与後の肺でのLXRの発現は、LXR-βは著変ないものの、LXR-αはブレオマイシン投与7日目をピークに約50％低下した。次に、LXRアゴニストによる治療効果について検討を行い、早期投与群（1日前～5日目）では線維化の軽減を認めたが、後期投与群（14日目～20日目）では改善を認めなかった。また、ヒト疾患肺を用いた検討は、良好な染色が得られず結論を出すに至らなかった。一連の研究の結果から、LXRが新たな治療ターゲットになりうる可能性があるが、マウスモデルで投与時期により治療効果に違いがある点など、今後さらなる検討が必要である。
