Target-controlled infusion (TCI) maintains predicted drug concentration of a drug calculated using a pharmacokinetic model. When propofol was infused at a targeted concentration for 30 minutes using a commercial TCI device which is available in Japan, measured plasma concentration of propofol during initial 2 minutes was >2 times higher than the predicted plasma concentration, whereas the measured concentration during the following 30 minutes was lower than plasma concentration, and also decreased gradually. Anesthetic awareness may occur if one refers the predicted plasma concentration of propofol at the loss of responsiveness during the anesthetic induction for the maintenance of anesthesia. The possibility might be reduced if one decreases initial infusion rate of the propofol TCI on the pharmacokinetic model of propofol by Marsh but not Schnider.
