Outline of Research at the Start

切除不能進行肝細胞癌に対し複数の薬剤が保険適用となっているが、薬剤ごとの奏効率には差異がある。遺伝子変異が規定する性状特性が奏効率差異に関与すると想定されるが、肝細胞癌の生物学的多様性、腫瘍内不均一性、時空間的な不均一性、遺伝子変異の関与を検出する現行法の実臨床上の忌避性、により治療前予測が未だ困難な状況にある。マルチパラメトリックMRIを用いてサイズ、形態、血行動態、性状特性、不均一性を含めた特徴を定量化し、精度の高い薬剤奏効率予測ができる可能性を検証する。

Outline of Annual Research Achievements

切除不能進行肝細胞癌に対し複数の薬剤が保険適用となっているが、薬剤ごとの奏効率には差異がある。遺伝子変異が規定する性状特性が奏効率差異に関与すると想定されるが、治療前予測が未だ困難な状況にある。一方、薬剤奏効率を予測する技術としてMRI画像への期待が高まっている。薬剤奏効率を予測可能なMRI画像所見の解析を目的として、まず、外科的切除された肝細胞癌症例を収集し、腫瘍血管構築、分子生物学的分類、免疫微小環境の観点から病理学的検証を始めている。具体的には下記の項目である。組織亜型、分化度、脂肪化、腫瘍血管構築(VETC pattern、Macrotrabecular pattern: CD34 IHCを併用)、分子生物学的分類(Wnt/β-catenin subclass(β-catenin核陽性and/or GSびまん性強陽性)、biliary/stem subclass(CK19+ and/or EpCAM+ and/or SALL4+)、p53 IHC、Ki-67 IHC、免疫微小環境(CD8, CD3, CD79a, Foxp3 IHC Immune-high/mid/low, TLS, Foxp3(Treg)/CD8 ratio, Foxp3(Treg)/CD3 ratio)。これら免疫微小環境を含めた病理組織学的所見と画像所見との関連性を検証し、病理所見から予測されうる化学療法薬の奏効率を予測できる画像所見解析を施行中である。また、実際に化学療法を施行された肝細胞癌症例の奏効率と、奏効率を予測可能な特徴的画像所見の解析を並行して進めている。

Strategy for Future Research Activity

現状の現状の症例収集による病理学的検証、実際の化学療法の奏効率と画像所見の解析をそのまま継続し、症例を蓄積していく予定である。
