Outline of Research at the Start

高齢心不全患者の重症化予防のためには、住み慣れた地域で周囲とケアを共創していくことが必要不可欠である。そこで、本研究の目的は、高齢心不全患者の重症化の社会決定要因を特定化し、さらに、変容的学習をとおした疾病管理と生活機能維持による重症化予防のための協働型の介入モデルを開発し有用性、妥当性を検証することである。以上より、在宅療養患者が「病いと共に生きる力」を育むQOL及びQODの実現と、地域共生社会における循環器病施策の推進を図る。
