Outline of Research at the Start

タンザニアでは妊産婦に栄養失調対策が急務であるが、栄養教育の重要性が十分に認識されず利用できる教材が極めて限られている。本研究では、タンザニアにおける妊産婦への栄養教育プログラム開発に向けて、妊産婦らや助産師らが取り入れやすいスマートフォンアプリ教材の開発と評価を目指す。はじめに妊産婦らを対象に健診成績、栄養状態、食慣習・栄養知識レベルなどに関する調査を行う。次に助産師らを対象に栄養教育の実践状況とニーズの調査を行い、現地の食文化や栄養状態に即したアプリ教材を開発する。さらにアプリ教材の使用感についてフィードバックを得ながら、妊産婦らが理解しやすく、医療従事者も取り入れやすい教材へと改良する。

Outline of Annual Research Achievements

タンザニアでは、妊産婦・乳幼児共に栄養失調対策が急務であるが、栄養教育の重要性が十分に認識されず、栄養教育に利用できる教材が極めて限られている。本研究課題では、タンザニアにおける妊産婦への栄養教育プログラム開発に向けて、効率的・効果的かつ妊産婦らや助産師らが取り入れやすいスマートフォンアプリ教材の開発と評価を目的とした。はじめに、タンザニアの妊産婦らを対象に健診成績、栄養状態、食生活などに関する調査を行う。次に助産師らを対象に栄養教育の実践状況とニーズの調査を行う。これらに基づいて、現地で入手可能な食材・強化食品の選択や、食生活の多様化の推進を通じた栄養教育教材を開発する。
2023年度は、プワニ州の2つのヘルスセンターで健診を受診する妊産婦約680名を対象とし食生活と健康調査を開始した。対象者の妊婦健診のデータを収集し、質問紙調査結果を含むベースラインデータを整備した。妊産婦の食生活、文化や風習を尊重しながら適切な栄養状態に導くためのツールが望ましいため、タンザニア人の公衆栄養学専門家を含む共同研究者などから、タンザニアの妊産婦の食文化に関する情報を収集していた。また、アプリ教材は動画コンテンツなど視覚的にわかりやすい素材を取り込める点や、携帯性・拡張性の観点から、低教育かつ医療施設へのアクセスが悪い妊産婦への栄養教育に有用と考えたため、企業と協力して、視覚教材や音声を導入できるアプリ開発に向けた協議を進めた。

Reason

当初予定していた妊産婦における食生活・健康調査が一年間をかけて順調にほぼ完了した。また、タンザニア国立食品栄養センター(Tanzania Food and Nutrition Center)の公衆栄養学専門家と連携しながら、栄養教育コンテンツの開発について情報収集を行った。

Strategy for Future Research Activity

2024年度からは、2023年度に収集したデータベースを整備し、タンザニア妊産婦の食生活と健康についての解析を行う予定である。解析結果については、栄養教育コンテンツの開発に向け、タンザニアの公衆栄養学専門家と母子保健現場の助産師を含む共同研究者らと討議する。また、県病院とヘルスセンターに勤める助産師と看護師を対象とし、周産期ケアにおける栄養教育の現状・課題とニーズを調査する予定である。
