Outline of Research at the Start

心臓突然死(SCD)は年間6-8万例発生し増加しているが、剖検を行っても約半数しか確定診断が出来ず、法医学上の極めて重要な課題となっている。近年、冠動脈に狭窄を認めない急性心筋梗塞(MINOCA)がSCDの原因の1つと推察されるも、死後のMINOCA診断は不可能で詳細は不明である。我々はMINOCAに特徴的な冠動脈病変を、光干渉断層法(OCT)を用いて報告した。本研究では、血管内光学的剖検用OCTプローベとそれを剖検で用いる手技を開発し血管内光学的剖検法を構築する。最終的にSCDにおいて血管内光学的剖検法を利用しMINOCAの診断およびその発症機序を解明する。
