| Outline of Research at the Start |
Wolff-Parkinson-White(以下、WPW)症候群は頻度の高い不整脈疾患である。ほとんどは孤発例だが稀に家族例が存在する。原因遺伝子としてPRKAG2、MYH7が知られているが、両遺伝子に病的バリアントを有さない家族例も報告されており未知の原因遺伝子の存在が示唆される。 研究代表者は近年、遺伝性WPW症候群を呈する家系においてミオシン関連遺伝子の病的バリアントを発見した。同遺伝子の病的バリアントによりWPW症候群を発症した報告はなく、新規表現型である可能性が高い。 本研究の目的は、該当バリアントを有するノックインマウスを用いて、WPW症候群の発症メカニズムを解明することである。

| Outline of Final Research Achievements |
Pathological variants of myosin-related genes identified in families with familial WPW syndrome and hypertrophic cardiomyopathy were knocked into laboratory mice using CRISPR-Cas9 technology for strain maintenance and phenotypic analysis. Hetero- and homozygous maintenance was possible, and analysis through electrocardiography and echocardiography showed no significant abnormalities in any of the indices, including PQ and QRS intervals and thickening of the ventricular septum and left ventricular free wall. There were also no differences in mortality or external surface malformations. Proteomic analysis using fetal hearts also showed no significant differences directly related to pathophysiology. These results suggest that this variant may not reproduce the phenotype in mice.

| Academic Significance and Societal Importance of the Research Achievements |
本研究では、家族性WPW症候群および肥大型心筋症の原因とされるバリアントを導入したノックインマウスにおいて、期待された表現型は確認されなかったが、これはヒトとマウス間の種差や発症機構の複雑性を示す重要な知見であり、今後の疾患モデル開発における方向性の検討材料となる。また、本研究で確立されたノックインおよびnullマウス系統は、他の解析手法やストレス負荷試験、加齢に伴う変化の評価など、今後の多角的な機能解析の基盤となるリソースである。さらに、発症に至らない遺伝子変異の存在を示した点は、将来的な遺伝子診断やリスク評価の精度向上にも寄与する。
