| Outline of Research at the Start |
本研究課題では、腹腔内において脂肪組織と悪性腹水が織りなす卵巣癌促進的土壌としての腹腔内エコシステムの本質に迫り、その治療標的化を目指す。具体的には新規超耐光性脂肪滴蛍光プローブと脂肪細胞モデルを用いた定量的な脂肪細胞脱分化評価システムを構築し、このアッセイ系により脱分化プロセスを抑制する候補物質を大規模化合物ライブラリーから探索する。更に、卵巣癌患者由来腹水の大規模コホートプロテオミクス解析から得られた知見に基づき、腹腔内への補体活性化阻害剤の投与が腹腔内微小環境改善に寄与する可能性についても検討を行う。

| Outline of Final Research Achievements |
To overcome the treatment resistance of advanced ovarian cancer with peritoneal dissemination, it is essential to develop novel therapeutic strategies that target not only cancer cells but also the tumor-promoting peritoneal microenvironment, including adipose tissue and malignant ascites. In this study, we developed a novel photostable lipid droplet fluorescent probe and established a screening assay to identify inhibitors of adipocyte dedifferentiation. Preliminary chemical library screening revealed that candidate compounds suppressed cancer cell proliferation via inhibition of adipocyte dedifferentiation. Additionally, proteomic analysis of ascitic fluid from 91 patients revealed three molecular subgroups and prognostic biomarker candidates. Functional assays using recombinant proteins demonstrated their inhibitory effects on cancer cell adhesion and proliferation. These findings support a new therapeutic approach targeting the peritoneal ecosystem.

本研究は、腹膜播種を伴う進行卵巣癌の難治性に対し、腫瘍細胞単体ではなく、脂肪組織や腹水などの腫瘍促進的腹膜環境全体を標的とする新たな治療戦略の可能性を示した点に大きな学術的意義がある。特に、脂肪細胞の脱分化を抑制する化合物のスクリーニングや、腹水中に含まれる予後関連分子の機能解析を通じて、腫瘍微小環境の改変による病態制御の概念を実証した。これらの成果は、従来の抗腫瘍治療では十分に対応しきれなかった播種性卵巣癌の新たな治療標的の同定につながるものであり、再発抑制や生存率向上への貢献が期待される点で、社会的意義も極めて高い。
