Outline of Research at the Start

REBOAは,動脈性出血制御と中枢臓器灌流維持を可能とする低侵襲な大動脈遮断法であるが,続発する虚血再灌流障害 (IRI) による酸化ストレスで,多臓器不全から死亡することがある.心停止後症候群や出血性ショックに対する水素ガス吸入の有効性・安全性が報告されている.本研究の目的はPost-REBOA IRIによる水素ガス吸入療法の安全性・有効性評価を行うことである.
ブタREBOAモデルで60分完全遮断を行った.遮断開始から2%水素吸入を行い,吸入有無で生存時間・乳酸値・酸化ストレスマーカーである血清8-OHdGを測定した.統計学的有意差はなかったが,水素吸入群で侵襲が軽減されている傾向であった.

Outline of Final Research Achievements

This study investigated the effects of 2% hydrogen gas inhalation on ischemia-reperfusion injury (IRI) induced by Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) in a swine hemorrhagic shock model. Ten swine were divided into two groups: the hydrogen group (n=5) received hydrogen gas during REBOA, while the control group (n=5) did not. Survival analysis showed no significant difference between groups (P=0.317). The hydrogen group exhibited a trend toward lower peak lactate levels (7.6±2.3 vs. 10.5±4.2 mmol/L) and lower serum 8-OHdG concentrations (0.11-0.19 vs. 0.12-0.32 ng/mL). Pathological analysis of the ileum showed no significant difference in injury scores (2.0±1.0 vs. 2.8±1.0). While hydrogen inhalation did not significantly improve outcomes, trends suggest potential protective effects that require further study.

Academic Significance and Societal Importance of the Research Achievements

REBOAに続発する虚血再灌流障害 (IRI) による酸化ストレスで多臓器不全から死亡することがある.心停止後症候群や出血性ショックに対する水素ガス吸入の有効性・安全性を受け,REBOA後 IRIにおける安全性・有効性評価を行った.ブタ出血性ショックモデルでのREBOA60分遮断に2%水素吸入を行ったが,水素ガス吸入は死亡率・バイオマーカー・病理変化をわずかに軽減する可能性はあるがIRI軽減に十分とはいえなかった。本研究のみで水素ガス吸入療法の有効性が完全に否定されるものではないが,REBOAの虚血再潅流障害を制御する新規研究は引続き望まれる.
