本研究では、がんサバイバーにとって発症前・診断・治療・回復までの様々な短期・長期的要素がその後の人生のアウトカムにどのような影響を与えるのか?という学術的問いを解明することを目的としている。がんサバイバーシップの複雑な要素と多様な人生のアウトカムとの関係を明らかにするために、①がんの発生・診断・治療・フォローアップの期間を通じた包括的データベースを前向きに構築し、②既存の大規模調査やデータベースを用いて、分析を行い、がんサバイバーにとって「がんとの共生」を実現しやすい環境をもたらすことが期待される。

| Outline of Final Research Achievements |
Advances in medical technology increased number of cancer survivors, and there is growing interest in post-treatment outcomes such as recurrence, quality of life (QOL), and employment. We analysed factors for survivors with diverse backgrounds to lead better lives after diagnosis. We constructed a multidimensional database using hospital-based cancer registries, DPC data, biobank lifestyle questionnaires, and distress screening. Using existing large-scale survey data, we found that survival was lower in deprived areas, pre-diagnosis smoking and alcohol consumption worsen prognosis, and younger individuals and those in part time job experienced financial toxicity. We will try to collect detailed assessment of QOL and subjective outcomes, and the development of evidence-based support through collaboration between clinical, nursing, and epidemiological fields.

| Academic Significance and Societal Importance of the Research Achievements |
本研究の学術的意義は、院内がん登録・バイオバンク・生活習慣アンケートを統合した多次元データベースを構築し、診断前後の生活習慣(喫煙・飲酒)や困窮度が予後や生存率に及ぼす影響を実証的に解明した点にある。これにより、ライフコース疫学の視点を導入し、多様な背景因子(年齢・雇用形態)が就労継続やQOLなどの「人生のアウトカム」に与える影響を示した。社会的意義としては、格差是正への政策的示唆(困窮地域の生存率低下)や苦痛スクリーニングや療養・就労両立支援の実態分析からサバイバーへの社会的サポートの基盤の重要性を提言した点にある。これにより、国のがん対策への実装を促進する研究への展開が期待できる。
