| Outline of Final Research Achievements |
Urinary and fecal incontinence, which is common among the elderly, can cause incontinence-associated dermatitis (IAD). We hypothesized that bacteria and fungi remaining in the skin contribute to the onset of IAD, which is insufficiently prevented by conventional cleansing and moisturizing care. Two objectives were set: (1) to clarify the composition of the bacterial and fungal microbiota present in the skin of diaper users and its association with IAD onset, and (2) to verify the preventive effects of interventions such as antimicrobial cleansing on normalizing the microbiota and preventing IAD. Due to the spread of COVID-19, the planned clinical study could not be conducted. However, we developed a microbiota analysis method focusing on live bacteria and analyzed fungal patterns in IAD-prone areas, achieving results that lay the foundation for future clinical applications.

| Academic Significance and Societal Importance of the Research Achievements |
本研究の意義は、皮膚の「生菌」に着目した新たな細菌叢解析法を検証し、viable PCR(PMA法)が生菌同定に適していることを示した点と、IAD好発部位にCandida属やTrichophyton属といった病原性真菌が残存することを示し、従来のケアでは除去困難な微生物がIADに関与している可能性を示唆した点である。これらの成果から、細菌・真菌叢を指標としたIADの予測検査法や、より効果的な予防法の開発につながる可能性があり、ひいては高齢者介護におけるIAD予防とQOL向上につながることが期待される。
