Outline of Research at the Start

リンパ浮腫患者の患部皮膚は蜂窩織炎の発症ならびにその再発のリスクが高いとされ,リンパ浮腫患者の最大の健康障害となっている.本研究では,未だ確立されていないリンパ浮腫患者における化膿レンサ球菌による蜂窩織炎に対する科学的根拠に基づく発症予防対策の策定を目的とする.そのために我々が作成したマウスの鼠径・膝窩・腸骨リンパ節を遮断することで下肢リンパ浮腫を発症するマウスモデルを基盤に,ヒトでの重症リンパ浮腫の症状を再現できるリンパ浮腫モデルマウスを作製し,同マウスを用いてリンパ浮腫への化膿レンサ球菌感染による蜂窩織炎の発症亢進の原因を明らかにする.

Outline of Final Research Achievements

Lymphedema is believed to develop due to lymphatic obstruction or tissue damage after surgery or radiation therapy, and the increased risk of skin injury and cellulitis in the affected area is problematic. In this study, we hypothesized that some kind of dysbiosis may occur in the bacterial flora composition of skin areas that have become lymphedematous due to surgery or radiation therapy, which may be the cause of the increased risk of skin injury and cellulitis. We constructed a mouse model of lymphedema that develops due to lymphatic tissue removal and radiation exposure, and examined the changes in the bacterial flora in the affected area. As a result, we found that the composition of the skin bacterial flora in the lymphedema-affected areas changed significantly, and the proportion of bacteria that may be harmful to the skin tended to increase.

Academic Significance and Societal Importance of the Research Achievements

外科手術、放射線治療、リンパ郭清術後のリンパ浮腫発症例が多数報告され、それが原因と考えられる蜂窩織炎の発症例が多く報告され問題となっている。しかし、リンパ浮腫による蜂窩織炎発症リスク増加の原因は不明であり、その原因を探索し、リンパ浮腫に続発する蜂窩織炎の発症予防対策を検討することが必要である。本研究ではマウスでのリンパ節郭清と放射線照射により発生したリンパ浮腫のモデルを用いてリンパ浮腫皮膚表層の皮膚細菌叢のディスバイオーシスを明らかにした。この結果は、リンパ浮腫患部への皮膚細菌叢のディスバイオーシス改善が蜂窩織炎発症抑制に貢献する可能性を示唆するものである。
