Outline of Research at the Start

褥瘡は難治性の皮膚傷害で、特に在宅において増加し、いかに在宅でも早期発見して早期治療につながるかが喫緊の課題である。本研究は先ず、早期褥瘡の病理機序の解明を行い、多循環障害の病態を突き止めた。同時にハンディタイプの簡易な早期褥瘡の診断機器の開発を進んで、無線通信手段や人工知能技術を活用して、在宅でも早期褥瘡の迅速、適確な診断システムの開発を4年間の計画で進行しているところである。

Outline of Final Research Achievements

The process of deterioration of an early pressure injury involves a combination of multi-circulatory disorders, including hyperemia, congestion, ischemia and hemorrhage, and the symptoms expressed on the skin can become unstable because of the changes in each condition. Because these changes are beyond the scope of application of existing clinical detections, diagnostic indicators with broad applicability are necessary. We have made it possible to individually evaluate various circulatory disorders in the skin by combining the general transparent disc method with the capillary refill time test employed in emergency medical care and using ultraviolet photograph capable of sensitively detecting hemodynamics. Among these, ischemia and hemorrhage are stable indicators. Comprehensive analysis of these conditions and ultraviolet photograph have increased diagnostic accuracy, making it possible to diagnose early pressure injuries over the entire process.

Academic Significance and Societal Importance of the Research Achievements

われわれは早期褥瘡の病巣に充血、うっ血、虚血、出血(多循環障害)の混在したことを発見し、臨床症状の不安定となる原因を突き止めた。この「多循環障害説」は従来の定説を覆し、これからの臨床診断と治療に対する意義が極めて大きい。特に除圧後虚血はこの学説の中核で、診断の安定した指標でありながら、除圧後の治療のターゲットともなり得る。この学説に基づいて開発したシステムは診断の正確性を高め、より早期の診断と治療を実現できることと、実験的人工知能によるデータ解析と自動診断も有効性を確認し、これからの応用で患者さんの苦痛を軽減するだけではなくて、医療費削減にも貢献できる。
