| Outline of Final Research Achievements |
This study aimed to (1) evaluate the effectiveness of a multi-user physical activity management system and (2) develop an algorithm for assessing physical activity and frailty using smartphone-based data. For (1), a multi-center study was conducted across acute care hospitals and community medical institutions, demonstrating that the system was particularly effective in maintaining physical activity and improving physical function during hospitalization in acute settings. For (2), a smartphone app was introduced through medical institutions and community organizations to more than 1,500 participants, enabling the collection of physical activity and frailty assessment data. The program was subsequently expanded to over 20 groups, including medical facilities and senior community associations. Mobility data obtained via the app was found to be useful in frailty classification, and combining it with age, sex, and BMI information significantly improved classification accuracy.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、慢性心不全患者の身体活動量と身体機能の長期的管理に対する多面的アプローチを通じて、臨床と地域をつなぐ新たな管理モデルの構築を試みた。入院中には多人数管理システムを活用した集中的介入、退院後はスマートフォンを用いた遠隔モニタリングにより、継続的な身体活動の支援が可能となった。また、地域の医療機関や団体と連携し、スマートフォンから得られるデータに基づくフレイルの早期把握や、地域環境と健康状態との関連についても検討を行った。これらの成果は、心不全患者に対する個別最適化された介入の実現と、地域包括ケアの質的向上に資するものと考える。
