Outline of Research at the Start

本研究では、特に申請者らが実績のある神経発達症と摂食障害に着目し、それらに対する本邦初のセルフヘルプ遠隔認知行動療法を開発して有効性を検証すると共に、その効果に対する脳機能メカニズムを解明することを目的とする。本研究の成果により、神経発達症・摂食障害に対する遠隔医療の仕組みを構築し、さらには、治療効果の客観的評価に資する新たな知見を得ることで、長期化、慢性化しやすい精神疾患の予防、早期改善につながることが期待される。

Outline of Annual Research Achievements

本研究は、神経性過食症に対するインターネット認知行動療法（ICBT）の有効性を検証する多施設ランダム化比較試験である。対象は13歳以上65歳以下の女性で、BMIが17.5以上の神経性過食症患者である。参加者は介入群または対照群に割り付けられた。本ICBTは12回の治療モジュールで構成されており、治療の有効性と安全性を介入前後で評価する。この研究は7つの医療機関で実施され、累計59症例が登録された。なお、この計画は2023年9月に英文誌に掲載された。

他方、ADHDを対象としたインターネット認知行動療法のプログラム開発を目的とし、この計画は2023年4月に英文誌に掲載された。コンポーネント・ネットワーク・メタアナリシスを用いて効果的な治療要素とその組み合わせを特定した。現在は解析結果を含めた内容について、英文雑誌への投稿を準備中である。また、利用者がオンライン上で治療プログラムを受けられるように、11回の治療モジュールを開発した。

Reason

摂食障害の症例登録数は予定を上回るペースで進んでいる。また、ADHDのプログラムに関しても、スウェーデンと連携し、プロトタイプが完成したため、順調に進展している。

Strategy for Future Research Activity

摂食障害のICBT研究に関して、残り1症例の登録を行う。有効性の評価は、治療開始時と治療終了時のEDEにおける客観的過食エピソードと排出行動の変化量により、実用可能性と治療効果を明らかにする。臨床試験に参加した研究協力者を対象に、長期的な有効性について検証するために、追跡調査を行う。さらに、ADHDのICBT研究に関しては、臨床試験のプロトコルを開発し、実施体制を構築する。
