Outline of Research at the Start

本研究は実臨床において一定のプロトコールの下よくコントロールされて実施された術前化学療法後に切除された膵臓胆道がんの化学療法奏功例と非奏功例におけるがんのゲノムや遺伝子発現、及び、ホストである患者のゲノム多型を解析することで化学療法感受性・抵抗性に関与する分子を抽出し、オルガノイド培養細胞の樹立、使用を含む機能解析を行なって化学療法感受性への関与を明らかにし、それらをバイオマーカーとする化学療法感受性予測、標的化による化学療法抵抗性克服法の開発を行うことで膵胆道がんの予後改善に寄与する。

Outline of Annual Research Achievements

1. 膵臓がん由来のがんオルガノイドを樹立し、原発腫瘍との比較を含めた詳細なオミックス解析を行って分子異常を指標とした標的治療実験を行い、また、同時にがん関連維芽細胞(CAF)を培養し、がんオルガノイドとの共培養モデルを作成して、空間トランスクリプトーム解析を施行し、がんオルガノイドとCAFとの相互作用をそれぞれの遺伝子発現変化から解析した。オルガノイドと組織標本の全エクソーム解析においてはKRAS, SMAD4, CDKN2A, TP53の膵がんの主なドライバー遺伝子の他、PRKACB等のプロテインキナーゼ、DUSP6などのフォスファターゼ、RNF43などのWNTシグナルに関連する遺伝子、SLC2A1などのトランスポーター遺伝子の変異が認められ、特徴的な遺伝子変異を有する膵がんオルガノイドを樹立することができた。これらで、SMAD4の変異パターンから合成致死を示す分子標的薬としてAurora A阻害剤の効果を検証し、SMAD4蛋白質短縮性変異を示すがんオルガノイドが特異的に合成致死を来すことを見出した。
2. 膵がんで活性化しているMAPK経路の標的遺伝子は膵がんの悪性形質、治療抵抗性、治療標的候補になり得るため、MAPK標的遺伝子群を明らかにし、その中でETV5に着目してその遺伝子発現機構を解析した。ETV5はMAPK活性に並行して発現変化し、それがMAPK作動性のETS-1のETV5プロモーターへの関与による事を示した。また、ETV5発現亢進が膵がんにおいて化学療法剤gemcitabineへの抵抗性に関与していることを明らかにした。
3. 膵がん切除後の残膵がん発生機構を解明するため全国施設から残膵がん再発例を集積しゲノム解析を行なって原発、再発がんを比較することで残膵がん再発機構を解明した。

Reason

当初計画で予定していた患者由来がんオルガノイドを用いた多角的解析を進めることができた。膵・胆道がんオルガノイドを使うことによりその遺伝子変異状態により合成致死をもたらすことができることを見出した。膵癌において活性化している代表的な信号伝達経路であるMAPK経路の下流遺伝子の遺伝子発現機構及びその機能を解析することにより、ETV5がMAPKで直接的に遺伝子発現調節を受けていることを示し、ETV5が膵臓がんの化学療法抵抗性に関与している事を見出した。残膵がん発生の分子機構を解明することができた。以上、当初目的に沿って膵・胆道がんの治療抵抗性機序解明に多角的にアプローチできている。

Strategy for Future Research Activity

患者由来オルガノイドを用いた多角的解析を中心に研究を続行していく。患者由来膵・胆道がんオルガノイドとCAFとの相互作用における分子発現関連の空間オミックス解析、マトリックス中での増殖、浸潤、粘液分泌機序の機能的解析、多角的オミックス解析に基づく膵胆道がんの病態解明と個別化分子標的治療法の開発を進める。
