Outline of Research at the Start

免疫微小環境の理解が膵癌患者の予後改善に必要である。しかし、数種類の免疫染色による従来法では、膵癌の複雑な腫瘍-免疫ネットワークの解明に限界があった。膵腫瘍1500例以上の多施設コホートにバンク化された膵癌組織検体を使用して、細胞レベルの超多重染色空間解析を行う。免疫細胞の数・位置・機能を定量化し、膵癌進行に関わる腫瘍-免疫ネットワークの特徴から新規治療ターゲットを探索する。治療・疫学データとの統合解析で、治療抵抗性をもたらす免疫学的因子や免疫賦活疫学因子を探索する。大規模コホートからの妥当性の高いバイオマーカーによる、個別化した膵癌免疫療法戦略を確立する。

Outline of Final Research Achievements

Pancreatic cancer is associated with a poor patient prognosis, but the immune response to the tumor plays a critical role in regulating survival. However, immunotherapies aimed at reactivating anti-tumor immunity have shown limited effectiveness in patients with pancreatic cancer, highlighting the need to identify factors that suppress the anti-tumor immune response. In this study, we employed a novel multiplexed immunofluorescence technique using 46 antibodies on approximately 500 surgically resected pancreatic cancer tissue samples. This method enabled the simultaneous identification of over 20 types of immune cells within the same tissue section. Our analysis revealed that the spatial distribution and density of immune cells, such as T cells and macrophages, varied significantly across patients, and these differences were associated with survival outcomes.

Academic Significance and Societal Importance of the Research Achievements

膵癌術後の患者さんにおいて、腫瘍内部に存在する免疫細胞の種類やそれらの細胞同士のネットワークが、患者さんの寿命を大きく左右することが明らかになりました。特定の免疫細胞の分布や相互作用が予後に密接に関係しており、これまでの治療では十分に改善されなかった理由の一端が解明されたと考えられます。今回の結果は、治療を受けていても再発や早期死亡につながる原因の理解に貢献するものであり、今後、新たな免疫標的治療や個別化医療の開発につながる重要な基盤データを提供するものです。膵癌の難治性克服に向けて、意義のある知見が得られました。
