骨髄異形成症候群(MDS)は造血幹細胞の異常に起因する難治性の血液がんである。その発症に関わる遺伝子変異は多数同定されたが、クロ ーン優位性の獲得と細胞死が共存する複雑な病態の発症機序は未だ解明されていない。申請者らはMDS患者の造血幹細胞・前駆細胞で過剰なミ トコンドリアの断片化が生じていることを発見し、これが炎症性シグナル経路の活性化をきたし、MDS病態の根幹となっていることを明らかに した。本研究では、ミトコンドリア断片化が炎症性シグナル経路活性化を誘導する分子基盤を明らかにし、前臨床試験を実施する。これにより MDS共通の病態が解明され、MDSに対する新規治療戦略の礎となることが期待される。

| Outline of Final Research Achievements |
Myelodysplastic syndromes (MDS) are refractory hematological malignancies caused by genetic abnormalities in hematopoietic stem cells. Although recent studies suggest non-infectious chronic inflammation is central to MDS pathogenesis, the precise molecular mechanisms remain unclear. We identified mitochondrial fragmentation within MDS clones, resulting in cytoplasmic release of mitochondrial DNA, which activates the cGAS-STING pathway via binding to cGAS. This activation leads to enhanced interferon production, thereby establishing a chronic inflammatory environment in bone marrow. These findings provide significant insights into the fundamental pathogenic role of chronic inflammation in MDS and are expected to facilitate the development of novel therapeutic strategies targeting this inflammatory process.

| Academic Significance and Societal Importance of the Research Achievements |
MDSは加齢とともに発症率が著しく増加する疾患であり、高齢化に伴い患者数は増加の一途をたどっている。発症年齢の中央値は70歳を超えており、多くの患者は造血幹細胞移植適応外となる。MDS病態形成における慢性炎症の重要性は指摘されていたものの、慢性炎症環境が形成される詳細な分子メカニズムは不透明だった。本研究の成果は、ミトコンドリア断片化と炎症性シグナル活性化の関連性を見出し、複雑なMDS病態の核心を解明した点で学術的意義が高い。さらに、慢性炎症を標的とした治療開発の新たな道を拓く成果であり、MDSの新たな治療戦略の開発につながる可能性を秘めている。
