Outline of Research at the Start

高齢者の健康長寿獲得には、自立した運動器機能が必須であり、それゆえ脆弱性骨折の原因である骨粗鬆症や虚弱をもたらす骨格筋萎縮(サルコペニア)の予防・治療が注目されている。骨格筋量と骨量には正の相関があることから、筋骨連関が相互の恒常性維持に重要といえる。このことから、本研究では、エストロゲンシグナルを基軸とした筋骨組織維持の分子メカニズムを明らかにする。

Outline of Annual Research Achievements

骨粗鬆症とサルコペニアは加齢とともに増加し、高齢サルコペニア患者の約60%に骨粗鬆症が併発する。近年、筋骨連関の作用が注目されているが、骨粗鬆症とサルコペニアの併発がどのように関連するかの詳細なメカニズムは未だ不明である。そこで筋骨連関メカニズム解明のため、ヒト骨格筋を用いてsingle cell RNA-seqを実施し、Linkage Disequilibrium Score Regression Analysisを実施した。その結果、骨格筋の間質に存在する間葉系前駆細胞であるPDGFRalpha陽性 (Pa+) 細胞の遺伝子発現プロファイルが、筋力のみならず骨密度と有意な相関を示した。さらに、その発現制御にエストロゲン受容体a (ERa) の関与が示唆された。これらより、Pα+細胞におけるエストロゲンシグナル欠乏が骨粗鬆症とサルコペニアの併発を誘因するという仮説を立て、Pa+細胞特異的ERa欠損雌マウスの骨および骨格筋の表現型解析を行った。Pa-CreER マウスとERa floxマウスとの交配によりPa+細胞特異的ERa欠損マウス (Pa-CreER;ERaf/f; cKO) を作出し、11-12週齢時にタモキシフェンを投与し、23週齢にて解析した。体重、子宮重量、脂肪重量、骨格筋重量および握力は群間で差を認めなかったが、大腿骨遠位部骨密度は、対照群と比較してcKOで有意な低下を認めた。マイクロCTによる骨構造解析では、海綿骨のパラメータには差を認めなかったが、大腿骨遠位成長軟骨板直下の皮質骨幅がcKOで有意に低下していた。骨形態計測では、cKOの皮質骨における骨形成率や破骨細胞数の有意な増加による高回転型骨代謝を認めた

Reason

Pa+細胞特異的ERa欠損マウスが、体重、子宮重量、脂肪重量、骨格筋重量および握力は群間で差を認めなかったが、大腿骨遠位部骨密度は、対照群と比較してcKOで有意な低下を認めた。また、マイクロCTによる骨構造解析では、海綿骨のパラメータには差を認めなかったが、大腿骨遠位成長軟骨板直下の皮質骨幅がcKOで有意に低下していた。

Strategy for Future Research Activity

今回の骨密度低下メカニズムを明らかにするため、骨格筋および骨組織からPα+細胞を単離し、RNA-seqによるエストロゲンシグナル解析を実施し、標的遺伝子による骨代謝調節メカニズムを明らかにする。
