Outline of Research at the Start

妊娠の場である子宮内膜において、着床を調節する機能的脂質は不明である。本研究では、着床における子宮内膜の機能的脂質を選出し、着床不全による不妊症の診断・治療のターゲットを探索する。着床不全マウスモデルとリピドミクスによる脂肪酸代謝産物の網羅的解析を利用し、着床に関わる子宮内膜の機能的脂質を同定し、その破綻による着床不全の病態形成機序について明らかにする。着床不全の病態の解明と治療法の開発に向けて、マウスモデルで得られた知見をヒト着床不全の子宮内膜の臨床検体を用いて検証する。将来的には着床不全のスクリーニングや新しい治療法への臨床応用が期待できる。

Outline of Annual Research Achievements

良好胚の胚移植反復不成功で定義される着床不全の診断・治療は未確立であり生殖医療の大きな課題となっている。本研究では、アラキドン酸経路の脂質が関わる着床不全の病態を調べるため、プロスタグランジン(PG)合成酵素であるシクロオキシゲナーゼ(COX)に着目した。着床子宮における2つのCOX(COX1、COX2)の機能を探るため子宮のCOX2欠損マウス(COX2uKO)、全身性COX1/子宮のCOX2の二重欠損マウス(DKO)を用いて着床期子宮におけるCOXの機能を調べた。COX2uKOの産仔数は正常なコントロールマウスの1/4程度に減少し、DKOでは産仔数0の不妊の表現型を示した。着床期子宮の解析により、DKOでは子宮の各種PG産生が減少し胚接着不全となるため不妊となることが判明した。一方COX2uKOは胚接着まで正常であったが、その後脱落膜化不全と胚浸潤不全を示し、胚浸潤期のCOX2uKO子宮ではPGE2およびPGD2が低下しており、PGE2誘導体またはPGD2誘導体の投与で脱落膜化と胚浸潤が救済された。着床期子宮において、胚接着をCOX1とCOX2が協同して制御していること、COX2がPGE2およびPGD2産生を介して脱落膜化や胚浸潤の過程を制御していることが明らかになり、子宮におけるCOXとPGによる着床制御機構の詳細を解明することができた。胚接着不全、胚浸潤不全、脱落膜化不全は、ヒトの着床不全の病因・病態と関連している可能性が考えられるため、COX・PG・その受容体を標的としたヒト着床不全の診断・治療への臨床応用に向けた研究の発展が期待できる。
