Outline of Research at the Start

本研究では金沢大学志賀町コホートに対しSNVジェノタイピングを行い、独自の遺伝的連鎖不平衡を加味したPRS算出スキームを活用し心不全多遺伝子リスクスコアPRSHFを算出するとともに、心不全ポリジェニックリスクスコアをリスクマーカーとしての有用性を評価し、さらには構成する各SNVを、介入可能な臨床形質座による分類・重みづけを行い、これにより、臨床パラメータとの関連から、疾患発症を予測するだけでなくどの項目に重点を置いて治療を行うべきかを明らかにする。

Outline of Final Research Achievements

Among the cohort of patients with hypertrophic cardiomyopathy, 267 HCM patients (64 with monogenic mutations and 203 without known causative mutations) participated alongside 133 controls and an additional 133 non-hypertrophic cardiomyopathy cases (also in the control group). After genotyping for the polygenic risk score for IVS thickness (PRSIVS) and performing imputation while considering linkage disequilibrium, information on approximately 3 million single nucleotide polymorphisms was obtained, based on GWAS data in Japanese individuals. HCM patients without known causative mutations showed higher PRSIVS compared to the control group, but the increase in PRSIVS was suggested to be associated with a higher lifetime risk of composite arrhythmic events only in patients with sporadic HCM.

Academic Significance and Societal Importance of the Research Achievements

原因変異陰性HCM患者は対照群と比較して高いPRSIVSを示すが、PRSIVSの上昇は単発性HCM患者においてのみ生涯複合不整脈イベントの増加と関連していることが示唆された。PRSIVSは、単発性HCM患者における不整脈リスク層別化のための貴重なツールとなりうるが、HCMの病因と臨床転帰における単発性因子と多遺伝子因子の複雑な相互作用を解明するためには、さらなる研究が必要である。
