Outline of Research at the Start

加齢,性差,循環特性,運動に対する血圧応答(運動昇圧応答)など脳血管シェアレイト応答に影響する生理要因を同定することにより,運動効果の個人差やばらつきの生理メカニズムを解明することを本研究の目的とする.脳血管シェアレイトに影響を及ぼす生理要因を調査し,特に脳血管内皮機能に影響する加齢,性差,血管特性(脳血管コンプライアンス等),運動昇圧応答の差異が脳血管シェアレイト応答に及ぼす影響を確かめる.これらの要因が脳血管シェアレイトに影響するとすれば,個人の生理的特性の差異により最適な運動が異なることが示唆される.

Outline of Annual Research Achievements

運動は脳循環機能を改善し、脳疾患や認知症の発症リスクを軽減することから、疾患予防の重要な手段と考えられている。しかしながら、運動による脳循環改善効果には個人差があり、さらに運動様式の違いによっても大きく変動するため、適切な運動処方の確立には依然として課題が残っている。本申請研究では、運動効果の個人差や運動様式による違いの生理学的要因を明らかにし、効率的に運動効果を得るための最適な運動処方の構築に資する有用な知見を得ることを目的として実験を行っている。特に、申請者がこれまでに行ってきた関連研究の知見をもとに、脳循環における運動効果のキーファクターとして運動に対する脳血管ずり応力(shear rate)応答に着目し、以下の2つの学術的問いに関連する研究課題に取り組んでいる。研究課題1)運動様式の違いによる脳血管シェアレイトの応答の差異が,脳血管内皮機能,さらに認知機能に直接的に影響するのか?研究課題2)加齢,性差,血管特性,運動昇圧応答等の脳循環動態に及ぼす生理要因の個人差が脳血管シェアレイトにどの様な影響を及ぼすのか? 本年度は本申請研究3年目にあたり、研究課題2に取り組み、特に運動効果の個人差を生じさせる生理メカニズムの解明を目的とした実験を実施した。その成果として、関連論文の発表(11編)に加え、脳血管shear rate応答に関する論文2編、および運動時の血圧応答の個人差と認知に関する論文2編を発表した。

2: Research has progressed on the whole more than it was originally planned.

Reason

関連研究は多数行っており、本年度は合わせて15編の論文を発表した。特に、研究課題に関する主要な研究として2つの実験を実施した。1つ目の研究では、運動昇圧応答の個人差が及ぼす影響を調査した。結果として、運動昇圧応答には個人差があり、特に過度な運動昇圧応答を示す個人では、運動による認知機能の改善効果が消失することが明らかとなった。この結果を学会で発表し、原著論文2編にまとめて発表した。2つ目の研究では、血管シェアレイト(shear rate)応答に関する検討を行い、以下の2つの主要な知見を得た。一過性の高炭酸ガス血症によって誘発される異なる程度のシェアストレスの増加に伴い、内頸動脈(ICA)のシェア依存性拡張が増加することを示した。しかし、ICAの拡張をベースラインからの総シェアストレス増加量で正規化すると、異なる高炭酸ガス血症レベル間での血管拡張の差は消失した。したがって、ICAの拡張を総シェアストレス増加量で正規化することで、脳血管内皮機能の評価の妥当性が向上する可能性があることを示した。また別の実験では、30分間のサイクリング(目標強度:換気閾値の80%)によってICAのシェアストレスが増加すると、一過性の高炭酸ガス血症に対するICAのシェア依存性拡張が促進され、脳血管内皮機能の改善を反映することを明らかにした。しかし、過換気(hyperventilation)によって運動誘発性のICAシェアストレスの増加を抑制した場合、この拡張の促進効果は消失した。これらの結果は、脳血管シェアストレスの増加が、若年成人において運動後の脳血管内皮機能の改善をもたらす主要な刺激となる可能性を示唆している。これらの実験は、2編の原著論文としてそれぞれ発表している.これらの知見は生理学的に重要であり、運動効果と関連する可能性があると考えられる。

Strategy for Future Research Activity

本申請研究において、研究課題1が完了し、本年度は研究課題2へ移行した。特に、運動効果の個人差に関わる生理メカニズム(脳血管シェアレイト応答の差異)の解明を研究課題2の目的として実験を進めてきた。現在、研究期間の4分の3が経過しているが、すでに研究課題2における運動昇圧応答の個人差に関する実験を完了し、論文発表に至っている。今後は、研究課題2のさらなる発展として、加齢や性差による血管特性および運動昇圧応答の違いが、運動中の血管シェアレイトにどのような影響を及ぼし、ひいては運動効果(血管内皮機能や認知機能など)にどのように関与するかを調査する。また、関連研究も引き続き進めていく予定である。
