Outline of Research at the Start

健康寿命延伸は本邦が持続可能な社会を実現するために重要な課題である。特に要介護の主要因である認知症は治療法が確立しておらず、病態機序に基づいた効果的な予防法の構築が重要である。認知症発症リスクは循環器疾患、特に動脈硬化に伴い増大し、一方で、習慣的な有酸素運動により低下することが先行研究から示唆されている。また最近の動物研究では、脳の老廃物除去機能低下が認知症の原因物質であるアミロイドベータ蓄積と関わることが報告されている。そこで本研究はヒト被験者を対象に、脳老廃物除去機能に対する有酸素運動の影響、さらにその生理機序解明のために動脈硬化の働きに着目し研究を実施する。

Outline of Annual Research Achievements

根治治療のない認知症の発症リスクは循環器系疾患、特に高血圧や動脈硬化の罹患により高まることが疫学研究から報告されている。しかし、その病態機序には不明な点が多く、有効な予防法を開発するための障害となっている。この背景を踏まえ本研究2年目にあたる令和5年度は、脳機能における「大動脈」の働きに着目し、運動習慣や脳循環との関連性を検証した。大動脈は脳と心臓を繋ぐ伸展性に富んだ血管であり、脳へ血液を送る導管としての役割と、心収縮によって生じる拍動を緩衝し末梢循環を平滑化するウィンドケッセル機能を有している。このウィンドケッセル機能の定量には幾つかの方法があるが、大動脈起始部における血流と血圧の同時計測から算出される大動脈インピーダンスはゴールドスタンダード指標だと考えられている。そこで我々は核磁気共鳴画像法(MRI)を用い、大動脈インピーダンスを非侵襲的に測定し運動習慣の影響を検討した(J Appl Physiol. 2024)。その結果、高周波帯域(4~8心拍高調波)における特性大動脈インピーダンスは運動習慣のある者では有意に低く、また一回拍出量や大動脈コンプライアンスと有意に相関することが示された。この結果は、運動習慣が心後負荷を低下し、脳へ伝わる拍動性を軽減、さらには、高血圧の発症リスクを低下することを示唆した。また令和5年度は、米国テキサス大学サウスウェスタンメディカルセンターとの国際共同研究を通し、脳循環生理学で著名なRong Zhang教授と共に脳血流自動調節機能の解析手法に関するエディトリアル論文を発表した(JCBFM 2024)。その他には米国(ACSM)、台湾(CARNet)、国内(日本生理学会)での学会シンポジウムでの発表、またコロナ禍より実施してきた脳循環に対する運動の急性効果を検証する実験を終了した。

令和5年度は研究計画書に記載されている課題2に関連する実験をおおむね終了した。現時点で本研究に関連する学術論文は臨床系および生理学系の国際誌(全てIF査読付き)に発表されている(令和4年度11件、令和5年度3件)。

Strategy for Future Research Activity

現在リバイス中の筆頭またはCA論文3編を出版し、令和5年度に終了した実験データをまとめ学会発表と論文投稿を行う。並行して研究分担者と進めているげっ歯類を対象とした動物実験データを纏め抄録を作成する。今後も継続して論文発表と学会発表、また地域住民への研究発表などを通じて学術成果を社会へ発信していく。
