Outline of Research at the Start

健康寿命延伸は本邦が持続可能な社会を実現するために重要な課題である。特に要介護の主要因である認知症は治療法が確立しておらず、病態機序に基づいた効果的な予防法の構築が重要である。認知症発症リスクは循環器疾患、特に動脈硬化に伴い増大し、一方で、習慣的な有酸素運動により低下することが先行研究から示唆されている。また最近の動物研究では、脳の老廃物除去機能低下が認知症の原因物質であるアミロイドベータ蓄積と関わることが報告されている。そこで本研究はヒト被験者を対象に、脳老廃物除去機能に対する有酸素運動の影響、さらにその生理機序解明のために動脈硬化の働きに着目し研究を実施する。

Outline of Annual Research Achievements

根治治療のない認知症の予防には習慣的な運動の実践が推奨されている。しかし、運動が脳機能を改善するメカニズムには未解明な点が多く、科学的根拠に基づくより効果的な介入法の開発が課題となっている。この課題の解決を目指し2024年度は、有酸素性運動および心肺機能の改善が加齢脳に及ぼす影響に関する複数の学術論文を発表した。まず、米国スポーツ医学会が発行する運動生理学分野の総説誌 Exercise and Sport Sciences Reviews において、生涯にわたる有酸素性運動の実践が心肺機能の向上を介して加齢による脳機能低下を軽減し、高齢期の認知症予防に有効であることを論じた(Tarumi et al., ESSR 2025)。また、米国テキサス大学サウスウェスタンメディカルセンターとの共同研究により、高い心肺機能が脳萎縮の抑制と関連し、さらに動脈硬化の抑制が脳白質における自由水分率の低下および脳萎縮の軽減と関連することを明らかにした(Won et al., J Appl Physiol 2025, J Magn Reson Imaging 2025)。さらに、運動を用いた脳血管機能評価法の開発にも着手し、若年健常者を対象とした実験において、周期的な低強度握力運動中の脳血流(経頭蓋ドップラー)および血圧を連続測定し、伝達関数解析を用いた脳自動調節機能の簡便な評価法を開発した(Qin et al., J Appl Physiol 2025, CA)。加えて、国際学会(米国スポーツ医学会)での抄録発表、臨床学術集会での招待講演、地域住民を対象とした講演活動などを通じて、運動の認知症予防効果に関する知見の社会発信にも注力した。現在は、脳老廃物除去機構に対する運動の急性・長期的影響を検証する研究を進行中であり、来年度の学会発表および論文化を目指している。

2: Research has progressed on the whole more than it was originally planned.

Reason

これまでに研究計画書に記載された課題1(動脈硬化が脳脊髄液[CSF]循環およびアミロイドβ[Aβ]クリアランスに及ぼす影響)および課題2(有酸素性運動がCSF循環およびAβクリアランスに及ぼす影響)に関する計測はほぼ完了しており、現在は学会発表および論文作成に向けた準備を進めている。さらに、課題3(神経認知機能とCSF循環・Aβクリアランスの関連性)についても、順調にデータ収集が進行中である。研究成果については、研究実績の概要で述べたとおり、積極的な発信を行っており、これまでに本研究に関連する論文を2022年度に11件、2023年度に3件、2024年度に7件発表している(いずれも国際誌・インパクトファクター付きの査読誌に掲載)。これらの成果は、本研究の学術的意義と進捗の順調さを示すものである。

Strategy for Future Research Activity

本研究課題の最終年度にあたる2025年度は、これまでに得られた計測データをもとに学会発表および論文作成を精力的に推進する予定である。現在、筆頭著者または責任著者として執筆した複数の論文が査読中もしくはリバイス中であり、研究期間中の受理を目指す。具体的には、スポーツ科学・生理学・臨床医学分野のQ1誌(例:Journal of Physiology、Medicine & Science in Sports & Exercise)への筆頭・責任著者論文3報、共著論文5報程度の発表を目標とする。さらに、引き続き国際・国内学会での発表に加え、地域住民を対象とした講演活動などを通じて、学術成果の社会還元にも取り組む予定である。併せて、米国の研究機関との共同研究を継続し、国際的な研究ネットワークの構築を図る。
