Outline of Research at the Start

実際の低酸素という現場は高所であるため、同時に外気温の低下も伴う(例えば、富士登山)。したがって、低温と低酸素という2つの環境ストレスに対する複合環境適応 (cross adaptation)こそがキーであり、従来生体適応の複雑さから、これら2つの環境ストレスに対する適応能は、個別に扱われることが多かった。人類が獲得した合理的な適応能力に関わる複数の生理機能(体温―循環(自律神経)―呼吸システム)に着目し、1)深部体温の変化、2)末梢血流の変化を誘発させ、さらに3)身体作業を負荷することで、富士山登山の出発から滞在までに起こる呼吸―循環―体温の身体システムを解明する。

Outline of Final Research Achievements

The respiratory-circulatory-temperature body systems that occur during exposure to cold and hypoxic environments were examined. In experiment 1, when skin temperature was progressively reduced under hypoxic conditions, the decrease in mean skin temperature induced an increase in blood pressure and cerebral blood flow from the vertebral artery, but cold-induced bradycardia developed. Furthermore, hypoxic ventilatory inhibition was accelerated. In experiment 2, the subject was loaded to lower-body negative and positive pressure loading under hypoxic conditions, and the baroreflex mechanism was confirmed, cardiac output was maintained, and mean blood pressure was also reflected in changes in cerebral and cutaneous blood flow. The effect of pressure loading on the hypoxic ventilatory response was small. In experiment 3, the walk-running transition speed was significantly faster in the cold and hypoxic environment, confirming an increase in energy metabolism in working muscles.

Academic Significance and Societal Importance of the Research Achievements

高地医学にも関連する新知見として、まず、高地登山時に起こる皮膚温の漸減を誘発した。皮膚温の漸減は熱産生を抑制し、換気低下を加速する。循環の心拍数が寒冷によって徐脈傾向になるが、血圧や頸部血流を亢進し循環機能を維持しようと防御作用がみられた。次に、低酸素では気圧の影響を受けやすく、これまでにみられた陰圧や陽圧に対する中心循環や脳血流の相乗応答が観察され、低酸素は過換気や徐脈を誘発し、うっ血や肺高血圧の防御作用ができていた。最後に、高地では作業強度が亢進し、低酸素寒冷は骨格筋代謝を亢進しているようである。登山から高地滞在までの身体システムの包括的な新知見を明らかにした。
