Outline of Research at the Start

本課題では,癌手術における主要なランドマークを客観的な観点から術中に教示するための技術開発を行う.癌に対する腹腔鏡手術を腫瘍学的・技術的に安全に遂行するためのCross AI navigation systemの実現に向け,(1)蛍光蛋白質を発現するレオウイルスを用いた癌進展範囲の術中診断,(2)人工知能を用いた手術操作における解剖学的ランドマークの術中教示の技術基盤をそれぞれ確立し,(3)それぞれの検証実験を行うことで,提案システムの臨床応用に向けたリスクアセスメントを行う.本研究の成果は,熟練外科医の暗黙知や膨大な病理標本のデータに基づく癌手術の情報支援システムを確立するものである.

Outline of Final Research Achievements

In this study, we developed the Cross AI Navigation System to enable real-time visualization of tumor localization and anatomical landmarks during cancer surgery, thereby facilitating intraoperative support. To achieve this, we conducted research on two key technical elements: (1) visualization of tumor progression ranges using fluorescent labeling with a reovirus, and (2) AI-based anatomical landmark guidance technology.As results, ① we created a genetically engineered reovirus expressing light-sensitive fluorescent proteins, which specifically infected gastrointestinal cancer cells, resulting in fluorescent visualization of the cells. Additionally, ② we developed a system capable of accurately recognizing and guiding multiple anatomical landmarks during rectal cancer surgery using AI software based on deep learning.

Academic Significance and Societal Importance of the Research Achievements

癌手術成績は解剖学的差異などの患者因子、または癌進展形式などの腫瘍因子といった様々な因子に影響されるが、手術の進行に伴って変化する術野において、術者が癌の進展範囲を正確に判断し、また臓器損傷を回避しながら、癌を露出することなく遺残のない切除を行うためには、極めて高度の技術を要する。
本研究は、腫瘍の可視化とランドマークの提示を融合することで、手術の安全性と精度を向上させる次世代医療支援技術を実現するものである。術者の経験に依存しない手術の均質化を促進し、地域医療格差の是正や国際的な外科教育の発展にも貢献する。
