Outline of Research at the Start

難治性てんかんの根治のためには脳内の発作感受性の消去が必要である。最近研究代表者らは、発作間欠期脳活動を用いた発作感受性の解読および定量化に成功し、さらにモデル動物において時空間特異的かつ細胞種特異的な深部脳領域刺激で発作感受性の低減・消去可能と見出した。しかしながら当該知見の応用には電極や光カニューラの脳実質への刺入に伴う侵襲性が課題である。一方、既存の非侵襲的脳刺激法は、深部到達性・空間特異性の不足および細胞種特異性のいずれかの欠如が課題である。そこで本研究は、非侵襲性、深部到達性、時空間特異性、および細胞種特異性を全て兼ね備えた新規脳深部刺激法の開発により、上記課題の解決を目指す。

Outline of Annual Research Achievements

難治性てんかんの根治のためには脳内の発作感受性の消去が必要である。最近研究代表者らは、発作間欠期脳活動を用いた発作感受性の解読および定量化に成功し、さらにモデル動物において時空間特異的かつ細胞種特異的な深部脳領域刺激で発作感受性の低減・消去可能と見出した。しかしながら当該知見の応用には電極や光カニューラの脳実質への刺入に伴う侵襲性が課題である。一方、既存の非侵襲的脳刺激法は、深部到達性・空間特異性の不足あるいは細胞種特異性の欠如いずれかが課題である。そこで本研究は、非侵襲性、深部到達性、時空間特異性、および細胞種特異性を全て兼ね備えた新規脳深部刺激法の開発による上記課題の解決を目的とした。当該新規脳刺激法と非侵襲的遺伝子導入法の融合により、革新的なてんかんの根治療法を創出する。さらにヒト頭皮上脳波から発作感受性を定量する手法の開発にも挑む。
2024年度は、小型集束超音波照射装置を用いマウスおよびラットの頭蓋骨を介した非侵襲的超音波ニューロモデュレーションを急性電気生理学的手法で達成した。次に2023度に見出した超音波ニューロモデュレーションに関わる分子を超音波遺伝学素子として深部脳領域における神経細胞に強制発現し、経頭蓋集束超音波照射により標的神経細胞の活動操作が可能であることを、組織学的および電気生理学的に証明した。さらにてんかん患者および非てんかん患者の頭皮上脳波記録データを収集し、発作感受性定量法の開発を目的とした解析を開始した。

Reason

本研究遂行に際して重要な技術的課題である「超音波遺伝学による脳深部領域における神経細胞活動の非侵襲的制御」を既に達成できているため。さらに、共同研究を通じて当初は想定していなかった超音波ニューロモデュレーションに関わるチャネル分子を新規に同定できたため。そのため「当初の計画以上に進展している」と判断した。

Strategy for Future Research Activity

2024年度までに、小型集束超音波照射装置の作出およびげっ歯類大脳神経細胞の超音波遺伝学的制御を達成し、ヒト頭皮上脳波の計測系の立ち上げおよびてんかんをはじめとする患者データの収集に成功した。2025年度は、当該成果の発展および応用に目指し、モデル動物において新しい非侵襲的遺伝子導入法および発作感受性消去法、ヒトにおいて頭皮上脳波記録を利用した発作感受性の定量法開発を推進する。
1. 非侵襲的遺伝子導入法:2024年度までに見出した超音波ニューロモデュレーションに関わる分子を超音波遺伝学素子として、新しい経頭蓋ソノポレーションで任意の脳部位に非侵襲的に遺伝子導入する技術を開発する。
2. モデル動物における発作感受性消去法の開発:海馬電気キンドリングラットにおいて、教師付き機械学習アルゴリズムの一種により定量化した発作感受性がコンベンショナルな抗てんかん薬で減少し、逆にてんかん誘起薬で増大するか検討する。
3. ヒト頭皮上脳波記録を用いた発作感受性定量法の開発:発作間欠期のヒト頭皮上脳波記録から発作感受性を定量する新技術を開発する。教師付き機械学習アルゴリズムの一種を用い、健常ボランティア被験者、てんかん患者、非てんかん患者各50人程度から常法に従って収集した発作間欠期かつ安静時の頭皮上脳波記録から、健常ボランティア被験者、てんかん患者、および非てんかん患者を定量的に区別する識別機を構築する。
