Outline of Research at the Start

本課題では、申請者が開発した拡散MRIの施設間差を調和するHarmonize法のバイアス補正能を臨床実装可能な水準まで改良し、神経変性疾患の大規模多施設MRIデータ(国内13施設1000例超)にこの新調和法を導入、神経変性疾患で早期に生じる神経突起、回路変化・神経炎症・脳クリアランス機能異常を評価可能な先端的拡散MRIと組み合わせ、神経変性疾患の臨床実装可能な早期診断バイオマーカー開発を行う。

Outline of Annual Research Achievements

拡散MRI(dMRI)は、精神・神経変性疾患の脳微細構造の変化の解明に大いに役立っている。しかし拡散MRI定量値は、 MRIモデル間の不一致による変動(施設間差)が生じるため広い臨床応用が妨げられている。 13種類のMRIモデルと2つのプロトコルで同一人物を複数の条件でスキャンした、69人の旅行被験者(TS)の計300スキャンのdMRIデータを用いて、diffusion tensor imaging (DTI)及びneurite orientation dispersion and density imaging (NODDI)定量値を算出、combined association test (ComBat)及びTS-based general linear model (TS-GLM)を用いて、調和前と調和後の評価を行った。その結果、ComBatとTS-GLMはともに、被験者間の年齢、性別といった生物学的効果を維持したまま、拡散MRI定量値のMRI撮像施設、モデル、プロトコルの影響を有意に減少させた。TS-GLMの調和力は、ComBatよりも強力であった。(Saito Y, Kamagata K, et al. さらに、大脳白質の線維束ごとのミクロ・マクロ構造変化を鋭敏に評価可能である拡散MRI技術であるfixel-based analysis (FBA)や、脳クリアランスシステムの非侵襲的指標として近年注目を浴びるALPS-indexも施設間差により変動することが知られているが、ComBatの適用により、いずれの指標においても施設間差を有意に低減可能であった。(Zou R, Kamagata K, et al. ISMRM. 2024; Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)

Reason

神経変性疾患の診断バイオマーカーとして有望とされるDTIやNODDI、FBA、ALPS-indexなどの先端的拡散MRI定量値における施設間差をComBatやTS-GLM法といった調和手法で低減可能であることを明らかとし、学会発表・論文発表を行った。(Saito Y, Kamagata K, et al. Aging Dis. 2023; Zou R, Kamagata K, et al. ISMRM. 2024; Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)
さらに、上記拡散MRI解析に関連した技術開発や(Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)、種々の神経変性疾患への応用(Tuerxun R, Kamagata K, et al. Front Aging Neurosci. 2024; Andica C, Kamagata K, et al. Mov Disord. 2023; Uchida W, Kamagata K, et al. NPJ Parkinsons Dis. 2023; Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)、これらの研究結果を含む総説論文を発表し、研究結果の普及に努めた。(Kamagata K, et al. Strategy for Future Research Activity

今年度の検討でComBat、TS-GLM法でDTI, NODDI, FBAやALPS-indexといった拡散MRI定量値の施設間差低減に有用であることが明らかとなった。そこで次年度より本調和法を、本邦で最大規模の精神・神経疾患の多施設研究であるBrain MINDS/Beyondデータベースや、国外の大規模多施設研究の疾患データ(アルツハイマー病:Alzheimer's Disease Neuroimaging Initiative, Open Access Series of Imaging Studies、パーキンソン病: Parkinson's Progression Markers Initiativeなど)に適用し、調和前後で疾患群-対照群間の拡散MRI定量値の効果量が改善するか、拡散MRI定量値と疾患重症度や認知機能などのスコアとの相関関係の変化などを明らかとする。
さらに調和法の適用により数百から数千といった大サイプルサイズを対象とし、神経変性疾患で生じる早期病理変化(神経突起変化・神経炎症・脳クリアランス機能低下など)を評価可能な先端的拡散MRI解析を組み合わせ、臨床実装可能な神経変性疾患のバイオマーカー開発を目指す。
