Outline of Research at the Start

本課題では、申請者が開発した拡散MRIの施設間差を調和するHarmonize法のバイアス補正能を臨床実装可能な水準まで改良し、神経変性疾患の大規模多施設MRIデータ(国内13施設1000例超)にこの新調和法を導入、神経変性疾患で早期に生じる神経突起、回路変化・神経炎症・脳クリアランス機能異常を評価可能な先端的拡散MRIと組み合わせ、神経変性疾患の臨床実装可能な早期診断バイオマーカー開発を行う。

Outline of Annual Research Achievements

本研究では、拡散MRIの先進的解析手法であるfixel-based analysis(FBA)において、多施設のMRIデータに起因するばらつきを補正するComBat harmonization手法の有効性を検証した。まず、同一被験者を複数施設で撮像した“traveling subject”(13種類のMRIモデルと2つのプロトコルで同一人物を複数の条件でスキャンした、69人の被験者(TS)の計300スキャン)データを用い、撮像装置やプロトコルの違いによって生じるFBA指標(fiber density, cross-section, FDC)の変動を評価したところ、ComBat適用後は測定誤差が大幅に軽減され、施設間での比較可能性が向上した。次に、アルツハイマー病患者と健常高齢者の多施設MRIデータに同手法を適用し、疾患関連変化の検出精度を評価した。その結果、ComBat適用後は、記憶機能と関連の深い脳弓(fornix)におけるFBA指標の有意な低下が明瞭に検出され、認知機能との相関も強化された。これらの結果から、ComBat harmonizationは、FBAの信頼性と疾患特異的変化の検出力を高める上で有用であり、多施設MRIデータを用いた脳画像研究の精度向上に寄与する手法であることが示された(Zou R, Kamagata K, et al. Reason

神経変性疾患の診断バイオマーカーとして有望とされるDTIやNODDI、FBA、ALPS-indexなどの先端的拡散MRI定量値における施設間差をComBatやTS-GLM法といった調和手法で低減可能であることを明らかとし、学会発表・論文発表を行った。(Saito Y, Kamagata K, et al. Aging Dis. 2023; Zou R, Kamagata K, et al. ISMRM. 2024; Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)
さらに、上記拡散MRI解析に関連した技術開発(Saito Y, Kamagata K, et al. Jpn J Radiol. 2023)や、種々の神経変性疾患への応用(Tuerxun R, Kamagata K, et al. Front Aging Neurosci. 2024; Andica C, Kamagata K, et al. Strategy for Future Research Activity

本年度の研究において、ComBat法およびtraveling subjectを用いたGLM(TS-GLM)法が、DTI、NODDI、FBA指標、さらにALPS-indexといった多様な拡散MRI定量指標における施設間差の低減に有効であることを明らかにした。これを踏まえ、次年度以降は、これらの調和法を本邦最大級の精神・神経疾患多施設データベースであるBrain MINDS/Beyondの実臨床データ、および海外の大規模研究(UKbiobank, ADNI、OASIS、PPMIなど)に適用する。具体的には、調和法の適用前後で、疾患群と健常対照群の間における拡散MRI定量指標の効果量がどの程度改善されるか、また各指標と疾患重症度・認知機能スコアなどとの相関関係がどのように変化するかを詳細に検討する。さらに、調和によって数百から数千例規模の統合解析が可能となることを活かし、神経変性疾患に特徴的な早期病理変化(例:神経突起変化、神経炎症、脳クリアランス機能障害など)に着目した先進的拡散MRI解析を実施する。これにより、病態の可視化と定量評価を通じて、臨床応用可能なMRIバイオマーカーの開発と、疾患の早期診断や重症度評価への貢献を目指す。
