Outline of Research at the Start

高齢者の増加に伴い、医療や介護に大きな負担がかかることが予測される。地域における医療介護連携の状況を継続的に分析・把握し、医療と介護が連携した効率的なサービス提供体制の構築が求められる。本研究では、地域の医療と介護サービスの提供状況を自治体が地域診断し、地域が求める医療・介護提供体制の構築に資するデータを得るための分析支援モデル（ツール）を開発する。自治体から提供を受ける医療・介護レセプト連結データベース（KDB）を活用し、地域における在宅医療の現状把握、退院後の療養場所の推移、在宅医療の自給率、在宅医療の需要推計等の分析方法を確立し、自治体が活用できる分析支援モデルとすることを目指す。
