Research Abstract

本研究は重症薬疹におけるTNFαをはじめとしたサイトカインの関与を検討するとともに、抗原提示細胞のアクセサリー分子の発現などを含めた免疫細胞の変化を検討した。
血清サイトカインに関しては薬疹型を播種状紅斑丘疹（MP）、多形紅斑（EM）、Stevens-Johnson症候群（SJS）、中毒性表皮壊死症（TEN）、薬剤性過敏症症候群（DIHS/DRESS）に分けて、各患者の発症期/急性期と回復期のサイトカインプロファイルを調べた。薬疹の急性期では全般的に炎症性サイトカインが上昇しており治療に伴って回復期に正常範囲に戻る傾向を示すこと、炎症性サイトカインの上昇の程度は必ずしも薬疹の重症度と相関しないことより、炎症性サイトカインは薬疹の病態形成には関与するが、発症機序においてはサイトカインだけでなく他の因子の関与が考えられた。
また重症薬疹の中でもSJS/TENとDIHS/DRESSでは異なるサイトカインプロファイルを呈した。SJS/TENと比較してDIHS/DRESSの急性期にはIL-5・IL-9・IL-13などのTh2サイトカイン、IL-10およびHMGB1が有意に上昇しており、DIHS/DRESSでは急性期にHMGB1がTh2細胞の活性化とHHV-6の再活性化に関与するとともに、Th2サイトカインとIL-10のバランスが病態形成に深く関わっている可能性が示唆された。
アクセサリー分子の発現の変化を含めた抗原提示細胞の数と機能に関しては薬疹の型および時期によって大きく変動していることが判明したが、詳細な検討は今後の課題であると考えられた。
