Outline of Research at the Start

変形性膝関節症(膝OA)は関節への過負荷と密接に関連し、関節構成体の変性・破壊を来す疾患である。近年、早期の病態解明から関節構成体の変化が軟骨摩耗より先行することが示され、パラダイムシフトが起きている。
我々は、骨棘、半月板異常、滑膜炎、軟骨下骨病変が膝OA進展の4大要因である可能性を示す中で、軟骨下骨病変は最強の進展因子であり、急速な関節破壊を来す膝疾患群でも大きく関与することに気付いた。
本研究では①大規模コホートの軟骨下骨病変と膝OA進展の関連、②膝OA患者の軟骨下骨病変の画像と病態、③脆弱骨マウスの実験的膝OA軟骨下骨病変解析から、軟骨下骨病変の病態解明と膝OA進展の意義を解明する。
