Outline of Research at the Start

糖尿病をもつ高齢者は、フレイル（心身機能の虚弱および生活機能低下）発症リスクが高く予後不良となりやすい。応募者らはこれまでに個別のリスクに応じる糖尿病者のためのフレイル予防ケアモデルを開発した。次の解決すべき課題は、専門外来に限らず幅広く糖尿病患者に質の高い予防ケアを提供することである。そこで本研究ではテレナーシングを活用した糖尿病フレイル予防ケアモデルの確立を目指す。大規模コホートを用いた地域在住糖尿病者のフレイルリスクの分析や、システマティックレビュー、専門家の意見収集を通してエビデンスの統合を行い、エビデンスに基づくケアモデルを開発し、効果検証を行う。
