Outline of Research at the Start	高齢者に対する「患者主体の移行期ケア」を促進するために、医療機関のスタッフに対する移行期ケアの教育プログラムを開発し、効果を検証する。このプログラムにより患者の自宅退院率向上・緊急受診件数の減少・在宅療養日数の増加を図るとともに、患者・家族の満足度向上、地域ケア機関との連携実績の向上、患者の退院前の準備状態の向上、医療機関スタッフの退院支援能力の向上、患者中心性の向上を図る。
