Outline of Research at the Start	人生の最終段階における看取り場所の理想と現実の非対称性は、過疎地域で大きく、終末期にある高齢者は自ら意思決定ができない状態であるため救急搬送件数が増加している。本研究では、私的・公的サポートが危機的状態にある過疎地域の要介護高齢者と家族介護者の意思の尊重とACP(Advance Care Planning;人生会議)に着目し、ソーシャル・キャピタルの蓄積により、住み慣れた場所で看取られる人生最終段階の「地域共生型意思決定支援モデル」の構築を目的とする。そのために、在宅要介護高齢者と同居の家族介護者を対象とした量的調査と質的調査の横断的調査の経年変化の把握・分析により、多元的に評価する。
