Outline of Research at the Start

心不全は増悪と軽快を繰り返しながら進行し、入退院を繰り返す疾患であり、高齢化により患者数はますます増加する傾向にある。心不全増悪による再入院率は高く、再発予防・再入院予防に向けた多職種チームによる多面的・包括的な疾病管理が求められている。心不全の再入院の主な原因として、塩分・水分制限の不徹底、治療薬服用の不徹底などの患者側の要因も多数報告されている。本研究では独自の電子的な心不全セルフケアツールを開発し、心不全患者を対象としたランダム化比較試験を実施することにより、心不全セルフケアツールが心不全の再入院の回避に有効かどうかを明らかにする。
