Outline of Research at the Start	我が国の高齢者介護施設、特に常勤医師を有し、薬物療法適正化の場として期待されている介護老人保健施設において、施設に関わる薬剤師と連携し、施設特性に合わせた処方ガイドを作成・導入する。処方ガイドの導入により、薬剤性有害事象や薬剤関連エラーなどの薬剤に関連した臨床アウトカムがどの程度改善するかについて、導入前と導入後のアウトカムを比較することで、処方ガイドの有効性を検証する。
