Outline of Research at the Start	近年、がん治療成績の向上に伴い治療後長期生存者（サバイバー）が増加しており、長期的な生活の質の向上が重要な課題となっている。医療機関でがん治療を終えたAYA世代のがんサバイバーは在宅療養へ移行（トランジション）するため、地域医療の中でフォローアップが可能となる看護支援の在り方を検討する必要がある。本研究では、地域にトランジションするAYA世代がんサバイバーに対する看護支援の現状と課題を文献検討とインタビュー調査により明らかとし、AYA世代がんサバイバーのトランジションを支える地域ケアモデルを開発する。
