人生の最終段階の医療・ケアについて、本人が家族等や医療・ケアチームと事前に繰り返し話合うプロセス (Advance care planning, ACP) は、「人生会議」として国がガイドラインで推奨し (厚生労働省, 2018)、医療現場では「病気の進行に伴い死が近づいている」、「積極的治療中止など治療方針の変化」などの難しい局面で、話合いがはじめられている (厚生労働省, 2022)。一方、ACPの改善に資する話合い実施状況や、モニタリング指標は、これまで示されていない。
本研究は、がん診療における人生の最終段階の医療・ケアについて、話合い実施状況と患者転帰のモニタリング指標を検討する。
